Visceral fat accumulation as an important risk factor for obstructive sleep apnoea syndrome in obese subjects.

چکیده
OBJECTIVES It is well known that obstructive sleep apnoea-(OSA) is frequently associated with obesity. In the current study, we investigated the correlation between abdominal visceral fat accumulation and the presence of OSA in obese subjects. SUBJECTS A consecutive series of 37 patients (17 men and 20 women) with primary obesity who were admitted to the Second Department of Internal Medicine. Osaka University Hospital, were investigated Patients with OSA were designated as those whose apnoea index (number of apnoea h-1 of sleep) was more than 5. MAIN OUTCOME MEASURES The distribution of body fat was determined using computed tomographic sections. The upper airway dimensions were evaluated with indices obtained by cephalometry in both inspiratory and expiratory phases. RESULTS Visceral adipose tissue (AT) area which was measured at the level of the umbilicus, and its ratio to total AT area were both significantly greater in OSA patients as compared with those in non-OSA patients. All subjects whose visceral AT area measured more than 220 cm2 manifested OSA. These two parameters also closely correlated with an increase in apnoea index. A multiple linear regression analysis revealed that the visceral AT area significantly correlated to apnoea index when age, AT mass and lean body mass were taken into account. The fluctuations of the upper airway were significantly greater in the large visceral fat group than in the small visceral fat group. CONCLUSIONS These results suggest that visceral fat accumulation is an important risk indicator for OSA in obese subjects.

Visceral fat and respiratory complications.
In the last 20 years, fat distribution has been considered a crucial causative factor of the excess morbidity and mortality found in obese patients. Several experimental evidences definitely linked visceral fat accumulation to metabolic disturbances [1–5]. The evaluation of fat distribution by the simple measurement of the waist circumference has been therefore suggested by guidelines to determ...

AIMS To determine the prevalence of sleep-disordered breathing (SDB) in a clinical sample of overweight and obese children and adolescents, and to examine the contribution of fat distribution. METHODS Consecutive subjects without chronic lung disease, neuromuscular disease, laryngomalacia, or any genetic or craniofacial syndrome were recruited. All underwent measurements of neck and waist cir...
متن کاملSleep apnoea and visceral adiposity in middle-aged male and female subjects.
In obese male subjects, visceral adiposity has been associated with obstructive sleep apnoea (OSA), while studies in overweight males and females are limited. Our goal was to examine the association between OSA and visceral fat in a relatively nonobese population and assess the effects of 2 months placebo-controlled continuous positive airway pressure (CPAP) use on abdominal fat. 81 subjects, 2...
متن کاملSites and sizes of fat deposits around the pharynx in obese patients with obstructive sleep apnoea and weight matched controls.
It has been suggested that deposition of fat in the soft tissues surrounding the upper airway may be an important factor in the pathogenesis of obstructive sleep apnoea (OSA) in obese subjects. We have used magnetic resonance imaging to determine the site(s) and size(s) of fat deposits around the upper airway in six obese patients with OSA (116-153% of ideal body weight) and five weight-matched...

With Obstructive Sleep Apnea Be Due to Obesity per se? To the Editor: I read with interest the recent report by Shamsuzzaman et al1 of elevated C-reactive protein (CRP) in patients with obstructive sleep apnea (OSA). Although the patients and control subjects were matched for body mass index (BMI), 36 4 kg/m versus 34 4 kg/m, all were very obese (upper normal BMI 25 kg/m). Several recent studie...
